resident ADMISSION
There continues to be a great demand for quality alternative living environments for our developmentally disabled population. These individuals need and deserve quality care in a safe and secure Christian environment. Chosen Vision’s Board of Directors is committed to serving this population.
We accept Resident Admission Applications on a continual basis to evaluate where the greater need is. Presently, there is a waiting list for placement in our homes. If you would like to place a loved one on our waiting list, complete the application, print and mail to Chosen Vision. The application is a downloadable word document.

We are currently seeking qualified applicants for our Direct Care Staff position(s). The Direct Care Staff is primarily a part-time position. This person is generally assigned a variable shift and needs to be willing to be available for any type of emergency that may occur while on duty. The Direct Care Staff person must be willing and desirous of working with developmentally disabled adults and be able to handle the physical demands that may be required to help with personal care tasks. The direct care staff reports to the House Manager.
